Reviewing and learning from the deaths of children and young people in Scotland
Scotland has a higher mortality rate for under 18s than any other Western European country, with over 300 children and young people dying every year. It is estimated that around a quarter of those deaths could be prevented.
Scottish Government requested that Healthcare Improvement Scotland and the Care Inspectorate work together to establish a system for reviewing and learning from the circumstances surrounding the deaths of all children and young people in Scotland. Based within the context of a National Hub, the programme aims to:
- ensure that the death of every child in Scotland is subject to a quality review
- improve the experience and engagement with families and carers, and
- channel learning from current review processes across Scotland that could direct action to help reduce preventable deaths.
The new local and national arrangements came into force on 1 October 2021 and will mean that for the first time in Scotland, reviews will be conducted on the deaths of all live born children up to their 18th birthday, or 26th birthday for care leavers who are in receipt of continuing care or aftercare services at the time of their death.

Eat Well Age Well Care at Home: Malnutrition Screening Pack
As part of Malnutrition Awareness Week from 11 October, Eat Well Age Well is launching their Care at Home: Malnutrition Screening Pack.
The Care Inspectorate is delighted to support the use of this screening pack and tools in the care at home sector as a good practice resource and approach to help care at home staff feel confident in understanding, identifying and addressing when a person’s diet is not meeting their nutritional needs.
The screening pack contains guidance along with tools and information to help you put this into practice. Eat Well Age Well also provides free training to support you to do this.

Safe Staffing Project - Open Badge now available
The Safe Staffing Project is working in collaboration with care service providers, care homes for older people and other stakeholders in health and social care partnership areas, to support the sector to be ready for the enactment of the Health and Care Staffing Scotland Act 2019.
The project team is delivering a series of quality improvement (QI) collaborative events, which develops the knowledge and confidence of participants in the guiding principle and duties of the legislation.
To enable participants to gain recognition of this informal learning opportunity, the project team, in partnership with the SSSC, has developed an Open Badge.
